Squint / Strabismus (click for worldwide facilities list): Strabismus or tropia are the medical terms for eye conditions commonly called by various names: eye turns, crossed eyes, cross-eyed, wall-eyes, wandering eyes, deviating eye, etc. Strabismus is not the same condition as lazy eye (amblyopia).

A strabismus is defined as a condition in which the eyes deviate (turn) when looking at the object of regard and a person can not align both eyes simultaneously under normal conditions. One or both of the eyes may turn in, out, up or down. An eye turn may called constant strabismus. (when the eye turns all of the time) or intermittent strabismus (turning only some of the time, such as, under stressful conditions or when ill). Whether constant or intermittent, strabismus always requires appropriate evaluation and treatment.

Strabismus is classified into many different types. Each type has its own causes, characteristics, and appropriate treatment plan. Most adults with strabismus have had the condition since childhood. However, strabismus can also begin in adulthood due to medical problems such as: diabetes, thyroid disease (Graves disease), myasthenia gravis, brain tumors, head trauma, and strokes. Occasionally, misalignment of the eyes can also occur after surgery on or around the eye, such as cataract surgery or retinal surgery, due to damage to the eye muscles during surgery.

A simple screening test for strabismus is the Hirschberg test. A flashlight is shone in the patient's eye. When the patient is looking at the light, a reflection can be seen on the front surface of the pupil. If the eyes are properly aligned with one another, then the reflection will be in the same spot of each eye. Therefore, if the reflection is not in the same place in each eye, then the eyes must not be properly aligned.

Advanced strabismus is usually treated with a combination of eyeglasses or prisms, vision therapy, and surgery, depending on the underlying reason for the misalignment. Surgery attempts to align the eyes by shortening, lengthening, or changing the position of one or more of the extraocular eye muscles and is frequently the only way to achieve cosmetic improvement. Glasses affect the position by changing the person's reaction to focusing. Prisms change the way light, and therefore images, strike the eye, simulating a change in the eye position.
